United Concordia Plan Types

United Concordia offers several dental product lines, each with different implications for denture and implant patients:
  • Concordia Plus DHMO plans, which feature low copays and require you to use an assigned primary care dentist. Dentures and other major services are available at fixed copay amounts.
  • Concordia Preferred PPO plans, which give you the freedom to see any dentist and include major restorative benefits at 50 percent.
  • Concordia Flex plans, which combine PPO and DHMO features.
  • TRICARE Dental Program (TDP), administered by United Concordia for the U.S. Department of Defense.
  • TRICARE Active Duty Dental Program (ADDP) for active duty service members.

Standard Commercial Plan Coverage for Major Services

Most United Concordia commercial dental plans cover:
  • Full and partial dentures at 50 percent, with replacement frequency every five to seven years.
  • Immediate dentures under standard prosthetic benefits.
  • Extractions and oral surgery covered at 70 to 80 percent under basic services.
  • Implants on enhanced plans, with coverage variations between plan tiers.
